Job purpose

Expert knowledge of care management operations and advanced knowledge of information systems and their application to care management. Works closely with the Director of Clinical Services, the Director of Business Intelligence, care management care teams, and the care management platform vendor to implement and maintain an effective technology solution that continues to evolve to meet care teams’ needs.  Responsible for working closely with the care management solution vendor initially in the design, testing, optimization, and training activities related to the new care management solution to ensure that the solution supports excellence in care management and end user experience. On an ongoing basis, the Solutions Lead will be responsible for leading training, content management, and quality improvement to ensure that the care management platform and other related technology enables high quality care management and allows the organization to meet accreditation standards.  

Duties and responsibilities

  • Implementation: Serves as the clinical project lead responsible for ensuring the integration of care protocols and workflows into a new care management platform, with support from the care management platform vendor and an internal team of care team member end users and data/analytics & IT staff. Serves as the primary Emtiro conduit to the care management platform vendor’s care coordination team for care management model questions during initial implementation and through ongoing operations. Supports implementation of additional related technology (e.g., patient-facing software applications) as needed.

  • Maintenance: Applies best practices to the management and development of the software life cycle and facilitates strategic clinical information system direction to support excellence in care management and end user experience. Serves as the primary lead working to gather, evaluate, and synthesize feedback from end users on system improvements and training needs.

  • Training: Leads training projects for care management platform and other technology, including the design, development, delivery, evaluation, and continuous improvement of training with direction and supervision from the Director of Clinical Services. Provides direct technical support for clinical and operational end users of the care management platform and other technology, as needed.

  • Quality: Drives best practice policy and standards with consistency and efficiency in a collaborative effort with the Director of Clinical Services to support program evaluation, staff performance evaluation, and to facilitate and maintain NCQA and other relevant accreditations.

  • Content Management: Assists with research to identify opportunities for new/improved content and application development. Designs processes and quality control programs around the creation and production of clinical content including processes to ensure all content is up-to-date and appropriately documented.

  • Collaborates with data/analytics and information technology staff to ensure clinical accuracy of clinical information in the care management platform and materials. Supports internal discussions as the knowledge expert for terminologies/nomenclature and how those are applied.

  • Graduation from an accredited School of Nursing (RN) with a BSN

  • Accredited School of Pharmacy (PharmD)

  • Master’s degree in Health Informatics, Public Health, or another relevant field. 

  • Experience in case management and/or care coordination preferred. 

  • Experience leading or substantially contributing to implementation or ongoing adoption and use of care management software and other technology.

  • Excellent oral, written, and interpersonal communication skills necessary to communicate effectively with the associates at all levels

  • In-depth care management software and operations knowledge, excellent working knowledge of clinical workflows, and working knowledge of the system development lifecycle

  • Technical computer skills and knowledge necessary to effectively use and support various software

  • Demonstrates a working knowledge of database applications and the skills needed to assist with testing plans and data

  • Ability to provide effective training

  • Demonstrates the leadership skills necessary to elicit cooperation and support from other associates

  • Detail oriented, organized individual able to manage multiple projects and service multiple stakeholders

  • Self-motivated, self-directed, and able to work independently with minimal supervision
